TITLE OF IMMEDIATE SUPERVISOR: Care Coordinator
JOB DESCRIPTION SUMMARY:
The Health Care Aide is a member of the homecare team who works under the supervision of the Care Coordinator and performs various personal care services as necessary to meet the client’s needs. The HCA is responsible for observing clients, reporting these observations, and documenting observations and care performed. The HCA will be assigned in a manner that promotes quality, continuity, and safety of a client’s care.
JOB REQUIREMENTS:
- HCA Certificate
- Alberta Health Care Aide Directory Number
- Valid CPR Certificate
- Police Information Check with VSC issued in the last 6 months from date of hire
- Immunization record for DTaP, MMR and Hepatitis B
- TB Clearance
- COVID-19 vaccination (at least first dose)
ESSENTIAL JOB FUNCTIONS INCLUDE, BUT ARE NOT LIMITED TO THE FOLLOWING:
- Provide client care as directed by the Care Coordinator/ Director of Care.
- Provide personal care.
- Assist with activities of daily living.
- Follow principles of infection control and Universal Precautions.
- Identifies and participates in performance enhancement activities.
- Participates in client teaching according to the client’s plan of care.
- Assist in providing a safe environment and promote quality patient care through adherence to established policies, procedures, and standards.
- Ensure client’s rights are adhered to.
- Respond rapidly and appropriately in an emergency situation.
- Use equipment properly and in a safe manner.
TITLE OF IMMEDIATE SUPERVISOR: Care Coordinator/ Director of Care
JOB DESCRIPTION SUMMARY:
The Licensed Practical Nurse is responsible for providing direct skilled client care under the supervision of a Licensed Practical Nurse/ Registered Nurse/ Director of Care. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the healthcare team to help meet positive client care outcomes.
JOB REQUIREMENTS:
- LPN Diploma
- CLPNA License Number
- Valid CPR Certificate
- Police Information Check with VSC issued in the last 6 months from date of hire
- Immunization record for DTaP, MMR, Hepatitis B
- TB Clearance
- COVID-19 vaccination (at least first dose)
ESSENTIAL JOB FUNCTIONS INCLUDE, BUT ARE NOT LIMITED TO THE FOLLOWING:
- Provides direct patient care as defined under the scope of nursing practice and the legislative acts using sound judgment and critical thinking.
- Implements plan of care and treatment.
- Provides accurate, detailed, and timely documentation consistent with the plan of care and the nursing process.
- Conducts ongoing assessments.
- Participates in health/therapeutic counseling, teaching, and emotional support to the client and family/caregivers.
- Observes client condition and does accurate reporting.
- Assesses and provides client and family/caregiver education and information pertinent to diagnosis.
- Prepares, administers, and appropriately documents actions taken specific to commonly prescribed oral, topical, subcutaneous, and intramuscular medications. Observes, reports, and records adverse reactions to medications and/or treatment to the registered nurse.
- Recognizes the need for and institute emergency measures in a rapid and appropriate and appropriate manner, when indicated.
- Participates in coordination of home health services, appropriately reporting the identified needs for other services/disciplines (i.e. PT, Dietician, etc.) to the Care Coordinator/ Director of Care.
- Performs all skills, treatments, and procedures competently in accordance to the provincial scope of nursing practice and adheres to the policies, procedures, and protocols of the agency.
